Responsibilities
- Deliver remote patient assessments and triage via telehealth platform
- Develop and implement individualized care plans for chronic disease management
- Coordinate with in-person care teams for seamless patient transitions
- Provide health education and coaching to patients through virtual consultations
- Maintain accurate electronic health records (EHR) documentation
- Participate in quality improvement initiatives and peer reviews
- Respond to urgent patient inquiries during designated on-call hours
Qualifications
- Active RN license in New Jersey or compact state
- BSN degree (MSN preferred)
- Minimum 2 years clinical nursing experience
- Telehealth certification or willingness to obtain within 90 days
- Proficiency with EHR systems (e.g., Epic, Cerner)
- Strong communication skills with cultural competence
- Ability to work independently with minimal supervision
- Valid BLS and ACLS certifications
