Responsibilities
- Deliver comprehensive virtual patient assessments and care coordination via telehealth platform
- Collaborate with interdisciplinary teams to develop personalized care plans
- Provide education and support to patients managing chronic conditions remotely
- Document electronic health records with precision and timeliness
- Monitor patient outcomes and adjust interventions as needed
- Participate in quality improvement initiatives for remote care delivery
- Maintain compliance with HIPAA and telehealth regulations
Qualifications
- Active RN license in Maryland or willingness to obtain immediately
- Bachelor of Science in Nursing (BSN) preferred
- 2+ years of clinical nursing experience
- Telehealth certification or willingness to complete within 90 days
- Valid US work authorization or eligibility for visa sponsorship
- Strong communication skills and proficiency with EMR systems
- Ability to work independently and manage virtual patient caseloads
- CPR/BLS certification required
