Responsibilities
- Provide remote patient assessments and telehealth consultations
- Coordinate care between patients, families, and clinical teams
- Monitor vital signs and medication adherence via digital platforms
- Document patient encounters in electronic health records (EHR)
- Collaborate with local providers for in-person care coordination
- Participate in continuous quality improvement initiatives
- Maintain HIPAA compliance in all digital communications
Qualifications
- BSN or Associate's Degree in Nursing (required)
- Active RN license in Kentucky or compact state
- Minimum 2 years clinical nursing experience
- BLS and ACLS certifications (or ability to obtain)
- Experience with telehealth platforms (preferred)
- Strong communication and critical thinking skills
- Eligible for US visa sponsorship (TN, H1B, etc.)
- Ability to work independently with minimal supervision
