Responsibilities
- Deliver high-quality patient care through hybrid in-person/remote monitoring
- Collaborate with interdisciplinary teams via telehealth platforms
- Develop and implement personalized care plans using digital health records
- Provide remote patient education and health coaching
- Participate in virtual case reviews and quality improvement initiatives
- Maintain compliance with HIPAA and telehealth regulations
- Lead innovative remote care protocols for chronic disease management
Qualifications
- BSN or Master's degree in Nursing (required)
- Active California RN license (or eligibility)
- Minimum 2 years clinical nursing experience
- Proficiency in telehealth platforms (e.g., Teladoc, Amwell)
- Experience with electronic health records (EHR) systems
- Strong communication skills for virtual patient interactions
- Ability to work independently with minimal supervision
- Eligibility for US work visa sponsorship
