Responsibilities
- Deliver high-quality patient care through in-person and remote telehealth monitoring
- Collaborate with multidisciplinary teams to develop and implement care plans
- Utilize EHR systems for accurate documentation and care coordination
- Provide health education and support to patients and families
- Participate in quality improvement initiatives and compliance audits
- Maintain patient confidentiality and HIPAA compliance standards
- Contribute to weekend/holiday coverage on rotating basis
Qualifications
- Valid California RN license or eligibility
- BSN degree (MSN preferred)
- Minimum 2 years clinical nursing experience
- Telehealth certification or willingness to obtain
- Proficiency in Epic EHR systems
- Excellent communication and critical thinking skills
- Ability to work independently and in team settings
