Responsibilities
- Deliver virtual patient assessments via telehealth platform
- Develop and implement personalized care plans remotely
- Coordinate with local providers for in-person follow-ups

- Participate in virtual case conferences and peer reviews
- Monitor patient outcomes through remote monitoring tools
Qualifications
- Active RN license in Tennessee (or willingness to obtain via compact)
- BSN degree (MSN preferred)
- 2+ years clinical nursing experience
- Telehealth certification or willingness to obtain
- Strong proficiency with EMR systems and telehealth platforms
- Excellent communication and critical thinking skills
- Ability to work independently with minimal supervision
