Responsibilities
- Provide virtual patient assessments and care coordination via telehealth platform
- Develop and implement personalized care plans remotely
- Collaborate with interdisciplinary teams through digital health records
- Conduct periodic on-site patient evaluations at Cincinnati facilities
- Monitor patient outcomes and adjust interventions as needed
- Document care accurately in electronic health systems

Qualifications
- Active RN license in Ohio or compact state
- BSN degree with 2+ years clinical experience
- Telehealth or remote care certification preferred
- Strong proficiency in telehealth platforms (e.g., Teladoc, Amwell)
- Eligibility for visa sponsorship (H1B/OPT candidates encouraged)
- Excellent communication and documentation skills
- Ohio BLS certification
