Responsibilities
- Coordinate patient appointments and healthcare services across Bay Area facilities
- Maintain accurate electronic health records using Epic systems
- Liaise between patients, providers, and insurance companies
- Assist with discharge planning and care coordination documentation
- Support patient education on treatment plans and community resources
- Collaborate with interdisciplinary care teams for seamless transitions
Qualifications
- Minimum 1 year healthcare coordination or medical office experience
- Proficiency with EHR systems (Epic preferred)
- California BLS/CPR certification required
- Exceptional communication and organizational skills
- Ability to thrive in fast-paced clinical environments
- Strong attention to detail and HIPAA compliance knowledge
- Reliable transportation for multi-site assignments
