Responsibilities
- Coordinate patient care plans and collaborate with multidisciplinary healthcare teams
- Manage patient appointments and ensure seamless transitions between care settings
- Maintain accurate electronic health records and documentation
- Provide patient education on treatment plans and preventive care
- Monitor patient outcomes and adjust care plans as needed

Qualifications
- Bachelor's degree in Nursing, Health Administration, or related field required
- Minimum 2 years of healthcare coordination experience preferred
- Current state nursing license or relevant healthcare certification

- Strong communication and interpersonal skills

- Valid driver's license and reliable transportation
