Responsibilities
- Provide high-quality patient assessments and care coordination via telehealth platforms
- Collaborate with interdisciplinary teams to develop patient care plans
- Document electronic health records accurately and timely
- Monitor patient progress and adjust interventions as needed

- Maintain compliance with HIPAA and state nursing regulations
Qualifications
- Active RN license in California or ability to obtain through sponsorship
- BSN degree (ADN candidates with 3+ years experience considered)
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care experience preferred
- Strong communication and documentation skills
- CPR/BLS certification required
- Ability to work independently and manage remote workflows
