Responsibilities
- Deliver high-quality virtual patient assessments and care coordination via telehealth platform
- Collaborate with multidisciplinary teams to develop and implement personalized care plans
- Provide remote medication management and patient education
- Monitor and document patient progress using our advanced EHR system
- Respond to patient inquiries and triage urgent situations appropriately
- Participate in continuous quality improvement initiatives
- Maintain strict HIPAA compliance and data security protocols
Qualifications
- Active RN license in the United States or eligible for immediate endorsement
- Minimum 2 years of clinical nursing experience
- Experience with telehealth platforms or strong willingness to train
- Excellent communication and critical thinking skills
- BLS/ACLS certification required (PALS preferred)
- Ability to work independently with minimal supervision
- Strong computer literacy and reliable home internet setup
