Responsibilities
- Provide remote patient monitoring and telehealth consultations for diverse patient populations
- Collaborate with multidisciplinary teams to develop and implement patient care plans
- Document clinical observations and care outcomes in electronic health records (EHR)
- Conduct periodic on-site assessments at partner healthcare facilities in Minneapolis
- Participate in quality improvement initiatives and evidence-based practice projects
- Coordinate care transitions between remote and in-service settings
- Maintain strict compliance with HIPAA regulations and telehealth protocols
Qualifications
- Active RN license in Minnesota or ability to obtain through compact endorsement
- BSN degree (MSN preferred) with 2+ years clinical nursing experience
- Valid US work authorization or eligibility for visa sponsorship (TN, H1B, O-1)
- Proficiency in telehealth platforms and EHR systems (Epic preferred)
- Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS)
- Experience with chronic disease management and patient education
- Strong critical thinking skills and ability to work independently
- Excellent communication skills with proficiency in English
