Responsibilities
- Conduct comprehensive patient assessments via telehealth platforms.
- Develop and implement individualized care plans in collaboration with physicians and specialists.
- Coordinate referrals and resources to ensure continuity of care.
- Document patient interactions and medical histories accurately and timely.
- Provide patient education and emotional support to improve health literacy.
Qualifications
- Active Registered Nurse (RN) license in the United States (or eligible for license).
- Eligibility for Visa Sponsorship (H1B, O-1, or Green Card process).
- Bachelor of Science in Nursing (BSN) preferred; Diploma or Associate’s degree accepted.
- Strong proficiency in electronic health records (EHR) systems.
- Excellent communication and interpersonal skills.
