Responsibilities
- Provide remote patient monitoring and telehealth consultations for chronic disease management
- Coordinate care between patients, specialists, and primary care physicians via our digital platform
- Document patient encounters accurately in Epic EHR system during on-site rotations
- Administer medications and treatments during scheduled in-person clinical days
- Educate patients on health management using interactive digital tools
- Collaborate with multidisciplinary teams to develop personalized care plans
- Participate in quality improvement initiatives for remote care delivery
Qualifications
- Active RN license in California (or ability to obtain through compact endorsement)
- BSN degree required; MSN preferred
- Minimum 2 years clinical experience in med-surg or telemetry
- Telehealth certification or willingness to complete company training
- Proficiency with electronic health records (Epic preferred)
- Valid U.S. work authorization or eligibility for visa sponsorship
- Strong communication skills with ability to build rapport virtually
