Responsibilities
- Coordinate patient care plans and ensure seamless transitions between healthcare services
- Collaborate with multidisciplinary teams to optimize patient outcomes
- Maintain accurate electronic health records and documentation
- Provide exceptional customer service to patients and their families
- Support healthcare providers in administrative and clinical tasks
- Monitor patient progress and adjust care plans as needed
- Comply with HIPAA and all healthcare regulations
Qualifications
- Associate's degree in Healthcare Administration or related field
- Minimum 2 years of healthcare coordination experience
- Certified Medical Assistant (CMA) or similar preferred
- Strong knowledge of healthcare systems and regulations
- Excellent communication and interpersonal skills
- Ability to thrive in a fast-paced environment
- Proficiency with EHR systems (Epic experience a plus)
- Valid Georgia driver's license and reliable transportation
