Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Manage electronic health records and documentation compliance
- Facilitate patient transitions between care settings
- Conduct telehealth consultations with flexible scheduling
- Maintain HIPAA compliance protocols
- Collaborate with insurance providers for authorization
- Train new staff on departmental workflows
Qualifications
- Bachelor's degree in Healthcare Administration or related field
- 3+ years clinical coordination experience
- Active AZ state healthcare license
- Proficiency in Epic EHR systems
- Certified Medical Coordinator (CMC) preferred
- Exceptional telehealth communication skills
- Ability to work independently with minimal supervision
