Responsibilities
- Provide direct patient care in assisted living and clinical settings
- Administer medications and treatments per physician orders
- Document patient progress and maintain accurate medical records
- Collaborate with RNs and healthcare team members
- Monitor vital signs and report changes to supervising staff
- Educate patients and families on health management
- Adhere to HIPAA and OSHA compliance standards
Qualifications
- Active Indiana LPN license
- Minimum 1 year clinical experience
- BLS/CPR certification
- Strong communication and documentation skills
- Ability to work flexible shifts including weekends
- Pass background check and drug screening
- Valid Indiana driver's license (for facility travel)
