Responsibilities
- Provide direct patient care in long-term and acute care settings
- Administer medications and treatments per physician orders
- Monitor patient vital signs and document clinical observations
- Collaborate with RNs and interdisciplinary care teams
- Implement patient care plans and maintain accurate medical records
- Educate patients and families on health management protocols
- Adhere to all state nursing regulations and facility policies
Qualifications
- Active KY LPN license required
- Minimum 1 year clinical experience preferred
- BLS/CPR certification (or ability to obtain within 30 days)
- Strong documentation and communication skills
- Ability to work rotating shifts including weekends
- Pass background check and pre-employment drug screen
