Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Conduct comprehensive health assessments and risk evaluations
- Facilitate transitions between care settings (hospital, home, rehab)
- Document all patient interactions in EHR systems with 99% accuracy
- Collaborate with insurance providers to ensure coverage compliance
- Develop personalized patient education materials
- Lead weekly case review meetings with clinical staff
Qualifications
- Bachelor's degree in Nursing, Public Health, or related field
- 3+ years of experience in healthcare coordination or case management
- Active FL RN license or certified Case Manager (CCM)
- Proficiency in Epic EHR and Microsoft Office Suite
- Fluency in English and Spanish (required)
- Strong problem-solving skills in complex patient scenarios
- Ability to manage 15+ active cases simultaneously
