Responsibilities
- Deliver high-quality virtual patient assessments and care coordination via telehealth platform
- Collaborate with interdisciplinary teams to develop personalized care plans
- Monitor and document patient progress using electronic health records (EHR)
- Provide health education and support to patients with chronic conditions
- Conduct periodic on-site health screenings in New Orleans community clinics
- Respond to patient inquiries and triage urgent cases per protocols
- Participate in quality improvement initiatives and peer reviews
Qualifications
- Active RN license in Louisiana or compact state
- 2+ years of clinical nursing experience
- Telehealth certification or willingness to obtain
- Proficiency with EHR systems (Epic, Cerner preferred)
- Strong communication and critical thinking skills
- Valid US work authorization or eligible for visa sponsorship
- BLS/CPR certification
- Experience with underserved populations preferred
