Responsibilities
- Coordinate patient navigation services for uninsured/underinsured Baltimore residents
- Conduct outreach in community centers, clinics, and local events
- Assist with Medicaid/Medicare enrollment applications
- Provide health education workshops on chronic disease prevention
- Maintain accurate documentation of client interactions and resource referrals
- Collaborate with healthcare providers and social service agencies
- Track program metrics and contribute to continuous improvement
Qualifications
- Associate's degree in Public Health, Social Work, or related field
- 1+ years experience in community outreach or healthcare navigation
- Certified Application Counselor (CAC) preferred
- Strong knowledge of Maryland healthcare systems and resources
- Exceptional communication skills in English and Spanish
- Ability to work flexible hours including evenings/weekends
- Proficient in electronic health record systems
- Valid driver's license with reliable transportation
