Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Manage electronic health records and documentation compliance
- Facilitate communication between patients, families, and medical staff
- Oversee patient admission and discharge processes
- Conduct regular care plan reviews and updates
- Collaborate with insurance providers for authorization
- Maintain HIPAA compliance in all patient interactions
Qualifications
- Bachelor's degree in Healthcare Administration or related field
- 3+ years of clinical coordination experience
- Strong knowledge of healthcare regulations and compliance
- Excellent interpersonal and conflict resolution skills
- Proficiency in EHR systems (Epic preferred)
- Valid Texas healthcare professional license
- Certified Medical Coordinator (CMC) certification preferred
